    New volunteers are sent the application form before training by the Volunteer Development Officer.

    This is only sent once VDO has spoken with the new volunteer and deemed suitable.

    This form should be completed as part of volunteers’ ongoing support. This gives the volunteer opportunities to raise any issues, concern and feedback at routine intervals.

    The Coordinator or Family Support Worker completes this every year with each of their volunteers.

    The ‘Volunteer Consents’ paperwork should be completed at the same time.

    This form is then completed by the most appropriate team member with the volunteer.

    This is an anonymous and optional form that volunteers can complete to help us gain understanding of our volunteer community.
