    Participant Consents are usually completed at the Home Assessment Visit.

    These should be completed annually with participants, or sooner if they change their mind about what they have consented to.

    All participants should complete an evaluation shortly after they join COVEY, which will act as their baseline.

    Another evaluation should then be completed after 6 months to track their progress.

    You can view any previously submitted evaluations in the ‘files and notes’ section of the participant’s Salesforce Record.

    Use this form to do the first risk assessment for new participants.

    This form is only for new participants joining COVEY, not existing participants who already have a risk assessment in place.

    When you submit this form, you will receive a unique link to your email address.

    Please save this link and use it to review and resubmit the risk assessment after 12 weeks (or sooner if required).

  • This consent form should be completed alongside introductory paperwork and renewed every 12 months. This consent should be in place before outings.

    Additional parental/carer consent must be obtained for activities involving additional/unusual risks. Staff must provide appropriate outing information and follow all relevant risk assessments, safeguarding and transport procedures.

    For school collections, staff should text the parent/carer to confirm collection and, where possible, provide an estimated drop-off time.
