Participant Consents are usually completed at the Home Assessment Visit. These should be reviewed and resubmitted updated once a year.

This form should be completed as soon as possible after a participant joins COVEY. This should then be reviewed and resubmitted every year.

This consent form should be completed alongside introductory paperwork and renewed every 12 months. This consent should be in place before outings.

Additional parental/carer consent must be obtained for activities involving additional/unusual risks. Staff must provide appropriate outing information and follow all relevant risk assessments, safeguarding and transport procedures.

For school collections, staff should text the parent/carer to confirm collection and, where possible, provide an estimated drop-off time.
